Documentation Required for Enrollment
The following information is required to enroll your child at Denair Charter Academy:
- Proof of Age
- Attendance records
- Discipline records
- Current Immunization record or Waiver Signed for Unvaccinated Student(s)
- Current transcript or most recent report card
- Proof of residence (utility bill)
- Custody papers (if applicable)
- Copy of IEP or 504 Documents (if applicable)
Special Notes:
- To be eligible for kindergarten, students must be 5 years of age on or before December 1st.
- Kindergarten students must complete a Dental Exam prior to first day of school.
- First grade students must complete and provide proof of a Physical Exam prior to first day of school.
- Grade 7-12 students need current Tdap shot.
- All students are required to have immunizations up to date prior to enrollment.
